A Warwick care home supporting people with learning disabilities has been stripped of its 'good' rating by the Care Quality Commission (CQC).
Following an inspection at the end of August, Individual Care Services' home at 60 Ward Grove has been rated 'requires improvement' after a number of issues were discovered by inspectors.
A report published last month showed the charity-run facility - which cares for up to three people at a time - had breached regulations in relation to consent, safe care and treatment, and good governance.
"The provider had not always worked within the principles of the Mental Capacity Act to ensure people's human rights were upheld," the report said.
"It was not always clear if people had been involved in decisions about their care."
It added: "Risks to people's health and well-being had been identified and assessed but not always managed safely."
The two inspectors who compiled the report also discovered records were not always kept of patients' care and support needs.
And they found a "significant incident" had not been reported to CQC when it should have been.
The facility, which did not have a service manager at the time of the review, was also found to show a "lack of effective oversight and governance" which meant people had "not received high quality, person centred care".
"Despite this, people appeared comfortable and relaxed in the presence of staff," the report added.
"Staff knew people well and understood their day-to-day role in protecting them from the risks of abuse.
"One staff member told us, 'safeguarding is about protection. Providing a safe space to vulnerable people who cannot always help themselves'."
